ME Saturday Program 2024/25

By filling this form, the parent(s) give(s) permission for Ihsan Muslim Explorers to photograph and video tape his/her child/ren for marketing*.Also, the parent accepts: I understand that it is my responsibility to update this form in the event that I no longer wish to authorize one or more of the above uses. I agree that this form will remain in effect during the term of my child’s enrollment. By signing below, I also agree that this is a legally binding form, and providing false information could be grounds for termination of childcare services, forfeiture of retainer, or both. Your selection below is to certify that you have read this consent form and indicates whether or not you give Ihsan permission to take appropriate photograph and video clips of your child. Ihsan may use the child’s photograph and video clips to promote its vision and programs.

* Ihsan respects privacy of all its attendees and supporters. Ihsan uses the photograph and video clips to communicate with the community. Ihsan uses photographs and video clips to promote its cause, vision and programs. Moreover, Ihsan also uses photographs and video to inform community of the effectiveness of its programs and garner more support.

It is extremely important that you provide the Student’s medical history or information about an existing condition, prescription medicine, or allergies.

In the event, reasonable attempt to contact me have been unsuccessful, I hereby give consent for a licensed physician to be called and administration of any treatment deemed necessary by the doctor.

1. Ihsan Muslim Explorers has every right to deny or suspend an enrollment of a student.
2. The tuition must be paid in full by cash, credit card or check in full or in different installments of this year as an early bird registration fee.
3. In case of unforeseen event, such as bad weather, Ihsan will notify parents through the email, WhatsApp group. Please make sure that your contact information is up to date.
4. Make-up days will not be given.
5. If your student is sick, (throwing-up, fever of 100. +, chicken pox, etc...) Your student CANNOT!!! be brought to the center under these conditions. If your student does become sick while at the camp, Ihsan will notify you to have the student picked up and an Illness report will be filled out, copied for student’s files, and given to you at time of pickup.
6. Prescription medications shall not be given to a student, except as authorized by a licensed Physician and with a written daily request from the parent or guardian. Non-prescription medication may be given to a student only with a written daily request from the parent or guardian. The medication should be in the original containers, with your student’s name and current expiration date.
7. Ihsan must have numbers where the parents or guardian can be reached. It is extremely important that these numbers be kept current, so that Ihsan may reach you when necessary, not just during emergencies.
8. Parents must sign an emergency medical release form. In the event of a medical emergency concerning your student, Ihsan will make every effort to contact you. In case you are not able to be reached, Ihsan must have these signed and kept on file in order to get medical care for your student.
9. Please notify Ihsan at once if you find out that your student has a contagious disease, so that Ihsan staff and the parents of the other scouts may respond appropriately. Ihsan will also notify you if your student has been exposed to any contagious illness.
10. Please inform Ihsan of any allergies, medical issues or dietary issues your student may have.
11. It is expected that each student be respectful to personal property of fellow students , Ihsan staff and Ihsan property. If your student has “maliciously” broken or damaged any property, you will be required to replace any object or reimburse Ihsan for its value.
12. Students are expected to follow teacher’s instructions and behave politely at all times. Disrespectful behavior will not be tolerated and the student will be suspended from the program if three separate disciplinary reports have been put in the student’s file or if the student “maliciously” injures another student, staff or Ihsan property.
13. Ihsan will not be liable for any injury or loss of property. In the case of an injury, an Injury Report will be filled out, copied for student’s files, and given to you at time of pickup.
14. If for any unforeseen circumstances you wish to withdraw your student (s) from the program, you will need to SUBMIT a written request to the program coordinator two weeks in advance and the prorated refund will be made from the date the request is approved.
15. Student must maintain regular attendance. Student who remain absent for three consequent Saturday camp without legitimate reason will be issued a warning letter. Student MUST maintain a 75% attendance level during the year.
16. A Parent must commit to helping with one or more of the festivals that the Muslim Explorers hold. A link in the website is designed to help facilitate a parent's effort.
